In-network insurance plans: 

 If in-network, we will bill your insurance on your behalf.  Any unpaid claims are the patient's responsibility.   

*I am not in-network with Cigna, Duke Basic, Duke Select, Medicaid, Blue Local, Blue Home, or Blue Value Plans. Upon request,  I can provide a receipt you may submit to your insurance for possible out-of-network reimbursement.  See FAQ for self-pay rates.

Questions to ask your insurance company:

  • Does my plan cover nutrition counseling code 97802?

  • Is a specific diagnosis required? 

  • How many visits are covered?

  • Do I have a co-pay or deductible?
